COMMUNITY HEALTH ACCESS PILOT PROJECT
Valley Vision is working on a project to inventory the relationships of health and mental healthcare providers in Sacramento County to understand the current nature of coordination, and design solutions such that underserved populations are more effectively served by the healthcare system.
Through identifying community-based healthcare providers, and understanding their various relationships, the system of care can deliver services, preventative or otherwise, that are not currently being provided in emergency departments.
Sutter Health Sacramento Region, Kaiser-Permanente, and Mercy asked Valley Vision to lead this project with a goal of providing underserved populations improved access to healthcare, while alleviating needed capacity in Emergency Rooms to serve more pressing and urgent needs.
High-Level Project Deliverables:
- A comprehensive inventory of assets and stakeholder relationships, with recommendations to foster greater collaboration among community healthcare service providers.
Specific Project Deliverables:
- A current and comprehensive database of primary and mental healthcare providers in the Sacramento County;
- A system that will be utilized by referral and intake personnel in healthcare organizations with a current and comprehensive inventory of primary and mental healthcare providers in Sacramento County; and
- Designed solutions and interventions to enhance collaboration among the community’s direct healthcare services providers, beginning with four specified ZIP codes in Sacramento.
