Safety net system development
Project Overview:
Sponsored by the region's four nonprofit healthcare systems, Valley Vision completed a Healthcare Community Needs Asssessment in the fall of 2010. A key finding of the two-year study was that the safety net system of healthcare operating in the region, though functional, was under-developed and lacked the structures needed to act in a coordinated, integrated fashion. To respond to this need, Sutter Health and Kaiser Permanente teamed up and asked Valley Vision to work to advance the degree of development of the safety net system functioning in Placer, Sacramento, Solano, and Yolo Counties.
Key Deliverables:
- Safety Net System asset mapping
- Assessments of the collaborative capacity of the safety net system
- Based on the results of the above assessment, design interventions that enhance the capacity of the safety net system to work collaboratively

Placer County - August 2010 through April 2011
Sacramento County - February 2011 through Fall 2011
Yolo County - Fall 2011
