Community Health Improvement Plan: San Joaquin County
Recognizing the need for targeted, coordinated priorities and goals to drive change, more than 100 stakeholders contributed to the development of the Community Health Improvement Plan for San Joaquin County. The CHIP was created to guide collaborative, long-term efforts to address community health needs throughout the county. Knowing that even a well detailed plan will not fully mobilize the actions desired, the Healthier Community Collaborative asked Valley Vision to help advance priorities, goals, and strategies outlined in the Community Health Improvement Plan (CHIP).
Valley Vision seeks to align efforts, as well as expand resources and capacity to address the priorities outlined in the CHIP. This is done by streamlining communication among stakeholders, analyzing assets and gaps, creating tools and guides for implementation, and promoting emerging funding opportunities. To ensure results, Valley Vision is encouraging common metrics that can be used across providers so that progress can be validated.
